First: Acknowledgment of receipt

I hereby acknowledge that I received from Smart Global Center for domestic worker Services,the Worker ____________________________ Nationality __________________________________________. The passport number is_______________________________ I pledge to return it to the center during the period of the agreed trial, and I also acknowledge with my knowledge that the domestic worker on the visa (tourist/sponsorship cancellation) and I took a note that the due date for fines is .................................. I undertake to take full responsibility for it from the receipt date indicated above and I undertake to return it immediately upon the probation trial period, upon request or in the event that it is not fit to work for me

Second: The Trail Period

I pledge to be fully aware that the trial period is only 3 days from the date of receipt of the domestic worker and is calculated against the worker’s operating fees during that period (AED 100 per day) and when the trial period is exceeded the operating fee (AED 200 from the fourth day until the date of delivery of the domestic worker to the center). Provided that the total duration of the trial shall not exceed seven days and in case of exceeding that period, I shall amend the legal status of the domestic worker and bear all the expenses of adjusting the status and the residency procedures as followed in the country and pay the total agreed recruitment fees.

Third: The pledges and obligations of the recipient (employer) as follows

I undertake to settle the status of the domestic worker in the event of acceptance after the trial period, and I undertake to settle her legal status and finish its procedures at my expense and under my sponsorship as required by the regulations of the State and the Ministry of Human Resources and Emiratisation within a maximum period of 7 days from the date of receipt. In case of failure to do so, I shall bear any fines incurred by the domestic worker by the ministry and the competent government authorities, and the original sponsor and the center have the right to take legal action and to circulate them. I am also not entitled to refer to the center and the original sponsor of any financial claims resulting from the violation of the terms of the contract or the laxity in amending the statues of the domestic worker

I undertake to return the domestic worker to the Centre in case of invalidity during the trial period specified above and to choose a replacement or return her after deduction for the service provided as specified in the trial period clause, and to recover the remaining amount paid in advance to receive the domestic worker, Within a maximum period of one week from the date of return of the domestic worker to the center

I bear any fees for the competent government authorities, whether it is visa issuance, status adjustment, transferring of sponsorship, residency extraction procedures or any fines incurred if I exceed the specified trial period and did not return the domestic worker to the center.

I undertake to bear the cost of treating the domestic worker at my expense, in case of illness or work injury during his time with me until he recovers and then delivers him to the center

I undertake to pay the operating fees of the center as specified in the test item from the date of receipt until the date of return, and in the case of return of the domestic worker after one month from the date of receipt and I undertake to amend the position of the domestic worker within the country under my personal sponsorship before returning him to the center

I undertake that in case of returning the domestic worker to the center, the visa cancellation period shall be at least 27 days, or bearing the amount of AED 50 fine for each day less than that period

Fourth: The terms of guarantee after transferring the sponsorship and the signing of recruitment contracts

The Center guarantees the domestic worker for a period of two years from the date of signing the recruitment contract, provided that the procedures for amending the worker’s visa status under the personal sponsorship of the employer are terminated in the following cases

Refusal to work – there must be convincing reasons for refusal to work due to the domestic worker himself and not to the employer or the nature of the work entrusted to him

Medical incompetence of the domestic worker – there must be a certificate issued and notarized by a government hospital

The employee escapes or stops working for his absence from the work

Fifth: Recruitment Fees

First Term “Trial period”

The trial period is 3 days from the date of receiving the domestic worker, and the second party undertakes to complete the remaining payment amounts or return the worker during this probationary period, otherwise the second party obligated to pay the employment fees at the rate of 100 dirhams for each day

Second Term “Delays and Fines”

The second party undertakes to pay any delay fines of 200 dirhams per day in the event of not return the worker to the center after the 3 days of the probation period or the delay in transferring or changing the worker’s status under his personal sponsorship in accordance with the UAE’s law and MOHRE Circular No

Third Term “During the warranty period”

The second party undertakes, in the event of returning the domestic worker, he must pay all salary dues and to cancel the visa or residence, with a grace period after cancellation not less than 27 days without ban, or he will pay AED 1350 as delay fines equivalent to 27 days
